We would like to set up a brief online meeting to tell you more about the scheme, so you can decide if it is right for you. To do this, we respectfully ask if you would please be willing to fill out the electronic form below.
We ask you for this information, because if you choose to get involved with the scheme, in due course we will use elements of this initial information to help us match you to a suitable mentee. The matching process between mentor and mentee is an art and not an exact science so we will also use our ongoing knowledge of you as we learn about you (and the mentee) in time to make the best possible matches. If you are linked to a mentee, we would only pass your contact details to them, with your full consent at that point and only when you are ready to connect with them at the end of the matching process.
The data you provide here is accessible to the scheme staff and will be stored securely in electronic format within our secure MyGateway online platform. Your form, when submitted, will also appear in the scheme manager's inbox and will then be stored in a dedicated named folder. In order for us to use your data (for example, your industry background, whether alumni of DMU, course of study and your motivation for mentoring) we need you to opt in as consent to do so. Therefore, on the form below we ask you for your permission to do so.
We will keep your contact information on file for a maximum of six years at which point you will be asked to resubmit a new form and data if you are still mentoring with us. Your old form will be deleted with us only retaining your first and last name as a record of your involvement in the scheme, should you choose not to fill out a new form to re-register. This is considered good practice and is an action that we have applied to the scheme to remain compliant with GDPR and quality assurance, which we have taken advice on.
